This form aims to collect relevant information regarding the buildings to decarbonise. This will allow PSDS Team to run the OnGen tool and generate a detailed report about the decarbonisation feasibility of the buildings involved.

This is a data collection form produced by the Midlands Net Zero Hub to gather information from clients to support building decarbonisation funding applications. The spreadsheet aims to collect relevant information regarding the buildings to decarbonise.
This form is composed of three tabs, each tab is one sub-form. Please complete one sub-form for each building and provide the attachments requested in section 4 for each building.
Please provide as much information as you can. The more information shared, the more precise the output can be.
There are drop down sections for additional information which can be revealed by clicking on the ‘+’ symbols on the left hand side.
- Please, use the ‘notes’ column to add specifications or relevant information.
- For Yes-No questions, please select your answer from the drop-down list.
- Please tick the attachments in section 7 to be included with the return of this form.
- The recommended format file for meter readings (attachments 1 & 2) is XLS (Excel).
